Significant Incidents at Fair Oaks Campus
Two separate incidents occurred at our Fair Oaks campus in December 2024 and June 2025. We acted immediately and strengthened safeguards across all campuses. We share the facts and what changed below.
December 2024 — Parking-lot vehicle collision impacting the playground
- What happened: After pickup, a driver experienced a medical event while reversing. A collision pushed into our playground. Five children and one staff member were injured. One child and one teacher died.
- Immediate actions: 911 called; first aid provided; area secured; roll call and classroom hold; families contacted; state licensing and law enforcement notified.
- Licensing review: State licensing reviewed the event and confirmed our team followed required protocols.
What we changed:
- Re-designed traffic flow; installed fixed barriers (bollards) between parking and outdoor spaces.
- Stopped using any play areas adjacent to traffic.
- Faster campus-wide alert protocol and parent-contact checklist.
- Targeted staff retraining on boundary supervision and emergency roles.

June 2025 — Classroom medical emergency
- What happened: A child with a known medical condition had a seizure and became unresponsive. Teachers started CPR, called 911, and contacted parents. The child passed away at the hospital.
- Immediate actions: CPR and first aid; emergency services on site; classroom secured; roll call; notifications to leadership and state licensing.
- Licensing review: State licensing investigated and did not cite violations.
What we changed:
- All-staff CPR/First Aid refresher completed.
- Scenario drills added for seizures and respiratory events.
